OSHA alleges Tulsa manufacturer exposed workers to safety, health hazards

TULSA, Okla. — A worker’s death at Accurate Manufacturing Inc. in April sparked three federal workplace safety and health investigations led to a plethora of safety and health hazards allegations.

Accurate Manufacturing is a Tulsa-based company that manufactures and builds steel products for various industries, including oil and gas. Accurate Manufacturing and it’s sister company Professional Fabricators, Inc. employ around 40 workers, according to the U.S. Department of Labor.

The Occupational Safety and Health Administration (OSHA) opened the investigations and allege the worker died from injuries sustained while pressure testing a heat exchanger.

“With pressure at 2,600 psi, one of the exchangers’ plugs blew off and struck the employee. OSHA determined the company violated the general duty clause for not keeping its employees safe,” said the U.S. Department of Labor in a press release.

Further investigations found 36 potential violations, including 25 “serious” safety violations.

The U.S. Dept. of Labor provided this list of violations found:

  • Failing to keep exit routes free from obstruction.
  • Allowing a barrel of flammable chemical to not be grounded and bonded.
  • Failing to have an energy-control program or provide related training.
  • Allowing the use of powered industrial trucks, slings and lifting devices that were not in safe operating condition.
  • Failing to install required machine guards.
  • Not ensuring electrical equipment was maintained properly and in safe operating condition.
  • Failing to provide flame-proof shields or screens and protective eye wear to welders and their assistants.

OSHA’s investigation of the facility also found 10 violations including failures to:

  • Employ a continuing and effective hearing conservation program and maintain accurate records of employees’ exposure measurements.
  • Establish and maintain an audiometric testing program and valid baseline audiogram.
  • Make sure the facility’s paint spray booth met all OSHA requirements.
  • Keep the space around the spray booth clear and install sprinklers.

Mequon Trinity-Freistadt site fire, history burns down

Fire destroyed part of Mequon’s Trinity-Freistadt Historical Site Monday, Aug. 28.

The site’s Mueller Shed stood for roughly 150 years only to burn down in one night. 

Freistadt is now officially part of western Mequon. The Trinity-Freistadt Historical Site preserves a unique part of Wisconsin history.

“It was founded in 1839 when Pomeranians came to the area. They came specifically for religious freedom,” said Terry Schoessow, the site’s co-president.

One of the site’s 12 buildings, the Mueller Shed was built in the 1870s and was used about a mile south of where it burned down.

“I thought it was a volcano in Hawaii or Maui or something,” Schoessow said. “I didn’t recognize it.”

Schoessow watched as firefighters worked and began their investigation into the cause.

“That’s still undetermined. We do know that the meter and main electrical connections for all the other buildings were in this building,” said Schoessow.

The fire burned and blackened basically everything in its path, but leaders hope there are some things they can salvage.

“The metal has made it through, of course, and then some small farm instruments – fanning mills, and plows and things – which we’re hoping made it through,” Schoessow said.

That hope remains as rubble covers history. The site is already receiving donations to rebuild whatever or however they can. Here’s how to help:

Northwest Biotherapeutics Provides Update On Near Term Plans For Submission of Marketing Authorization Application

BETHESDA, Md., Oct. 13, 2023 /PRNewswire/ — Northwest Biotherapeutics (OTCQB: NWBO) (“NW Bio”), a biotechnology company developing DCVax® personalized immune therapies for solid tumor cancers, provided an update today about its progress toward submission of a Marketing Authorization Application (MAA) to the Medicines and Healthcare Products Regulatory Agency (MHRA)(the equivalent of the U.S. FDA) in the U.K. for commercial approval of the Company’s DCVax®-L treatment for glioblastoma.

The Company reported that the majority of the MAA has been completed, and all but one of the key sections of the MAA have been delivered to the publisher. The publisher is an independent party who does the final step of the preparation for the submission: formatting, checking references and the like. 

The Company and its consultants are working intensively to finalize this last key section of the MAA, after overcoming an unexpected delay in that regard. The Company strongly believes that after so many years of work on the DCVax-L program, taking some additional time to help ensure that the full MAA package is as strong as it can be is especially important since the Company plans to submit applications to multiple regulators.    

An additional factor in the timeline is that certain expert consultants who are key to this remaining section will be temporarily unavailable during the next two-week period, returning the week of October 30.  The Company anticipates that the remaining section of the MAA package will be delivered to the publisher within about two weeks after the consultants return.
